Advanced Directives
An Advanced Directive is a legal document that you fill out to let
others know what to do in case you cannot make health care decisions
for yourself. Below we have listed two of the more common types
of Advanced Directives.
-
A Living Will is a directive intended to protect your wishes
in the event you are unable to let them be known. This document
delineates your wishes regarding any limitations on medical treatment
you may desire and to what extent life support measures should be
utilized.
-
A Durable Power of Attorney for Health Care is a legal signed
document that names another person to make health care decisions
for you in the event you are unable to make them for yourself.
Please dial 719-587-1350 to speak directly to our Patient and Family
Services representative if you have any questions regarding a living
will or a durable power of attorney.
