SLVRMC BENEFIT SUMMARY

Health Insurance: SLVHMO: TPA - Self Funded Plan

Plan 1
Family $830 / month- Hospital contributes $498 employee contributes $332
$700 deductible, 20% co-pay up to maximum amount per year .
Most to come out of pocket is $2700 for a plan year, then insurance pays 100%

Single
$ 364/month - Hospital contributes $219, employee contributes $145
$350 deductible, 20% co-pay up to maximum amount per year .
Most to come out of pocket is $1350 for a plan year, then insurance pays 100%

Plan 2
Family $572 / month- Hospital contributes $342, employee contributes $230
$2000 deductible, 20% co-pay up to maximum amount per year .
Most to come out of pocket is $7000 for a plan year, then insurance pays 100%

Single
$ 310/month - Hospital contributes $187, employee contributes $123
$1000 deductible, 20% co-pay up to maximum amount per year .
Most to come out of pocket is $3500 for a plan year, then insurance pays 100 %

Pharmacy $10 Generic copay or greater of $20 or 20% coinsurance for Brand (Prescription Solutions)

High Option
Covers first $160.00 per plan year.

Life Insurance
Term life Insurance equal to annual salary, paid by employer.
Long Term Disability, paid by hospital.
Option for employee to purchase additional life insurance and dependent life insurance at reasonable cost to employee.

Cancer Insurance
Supplemental to your regular medical care benefit. Option to purchase for medical expenses for cancer treatment, employee paid.

Pension
Hospital pays entire contribution, at the rate of 6% or gross annual income. Employee enrolls after 2 years of service and is 100% vested. Employees who work 1000 hours per year are eligible for enrollment.

FICA
SLVRMC pays 7.5% of each employee’s annual salary.

Tax Deferred Annuity
Retirement plan that the employees contribute to on a pre-tax basis, all employee eligible upon date of hire.

Paid Days Off (PDO)
Less than 5 years employment, earn 168 hours/year (21 days). Over 5 years employment earn 208 hours/year (26 days) up to a maximum of 240/hours.

Extended Sick Leave
Earn 56 hours year (7 days)

Employees who are insured under our plan, get a 50% reduction in balance due after insurance payment. Employees in other comparable plans will receive a 50% discount if the plan is equivalent to SLVRMC’S

Liability Insurance
SLVRMC pays 100% of the cost of liability insurance for each employee.
