SCHOLARSHIP
FUND FOR HEALTHCARE PROFESSIONAL EDUCATION
Purpose
San Luis Valley Regional Medical Center Fund for Healthcare Professional
education was created in recognition of Anthony Amato and in memory
of Claudia Bradshaw Lopez. The Scholarship Fund was established
to provide funds for all health care professionals.
The award shall be for one academic year, and if deemed advisable
by the selection committee, can be extended to two, three, or four
years. The scholarship will include any accredited school, in state
or out of state
Policy Statement
The Lutheran Hospital Association of the San Luis Valley (the Board
of Trustees of the San Luis Valley Regional Medical Center) shall
have total control over the Scholarship Fund. They shall exercise
such control by appointing a Scholarship Committee for the Scholarship
Fund for Healthcare Professionals whose duty it will be to administer
the affairs of the Scholarship Fund. Such committee shall consist
of not less than five (5) or more than nine (9) members, two of
whom must be members of SLVRMCs Board of Trustees.
The Board of Trustees shall appoint two representatives from their
board. The hospitals administrative team will appoint the
remaining seven members to the Scholarship Committee. All members
shall serve a minimum of 1 year and a maximum of 2 years. The individuals
will continue to serve except when resignations are requested by
the Board of Trustees or hospital administration for lack of interest
shown by not attending scheduled meetings.
Replacements on the Scholarship Board shall be recommended by the
Scholarship Committee and hospital administration and approved by
the hospitals Board of Trustees.
Should this Scholarship Fund be discontinued for any reason unknown
at this writing, the principal sum and accrued interest shall revert
to the restricted funds of the Lutheran Hospital Association of
the San Luis Valley.
The principal fund may never be depleted but is to serve as a base
fund to be invested to produce the most revenue. Earned interest
will be used for payment of scholarship awards. The amount of money
available in the form of accrued interest will determine the amount
and number of scholarships to be made available during any one year.
The Scholarship Board shall make all decisions concerning the use
of such funds, establishing priorities for manpower needs in the
Allied Health field.
All subsequent donations will be added to the principal fund. All
interest received shall be invested in money market account for
easy withdrawal.
Application Process
Applications must be completed and received by the 2nd Friday of
April each year to be considered for the next years scholarship.
The applicants will be reviewed by the end of April each year. The
recipient names will be announced before the last week in April.
The award shall be for one academic year at a time. If deemed advisable
by the Scholarship Fund Committee, the applicant may reapply each
year up to a total of four years depending on the length of their
particular healthcare education program. The scholarship can be
applied to any accredited college or technical school anywhere in
the United States.
All applications are to be sent to Clorinda Chavez, Director, Human Resource Deaprtment
at SLVRMC. An anonymous presentation of each applicant will
be made to the Board for selection of the candidate.
- The
applicant must be a high school graduate and presently residing
in the San Luis Valley. This will include Costilla, Mineral, Saguache,
Alamosa, Rio Grande, and Conejos Counties.
- The
scholarship will be offered to male or female applicants regardless
of race, color, religion, national origin, age, sex, disability
or veteran status.
- The
applicant must have been in the top fifty percent of their high
school graduating class.
- The
applicant must have applied and been accepted for admission to
an Allied Health program in an accredited school, within Colorado
or out of the state.
In addition the applicant must include:
- A
letter of acceptance into an accredited program,
- A
letter from the student stating why the student would like to
be considered,

Requested personal information,
- Two letters of reference from the students current school
(high school or college), or one letters of reference from current
employer and one reference from community organization,
- The
applicants latest grade point average.
Procedure for Repayment
The recipient will be required to complete one year of employment
at the San Luis Valley Regional Medical Center for each year they
receive a reward. If a position is not available at SLVRMC, or the
student chooses to work at another facility, the repayment process
begins 6 months after graduation with a repayment schedule agreed
upon by the committee and the student via contract.
If a student drops out of any Allied Health program or is not allowed
to continue because of poor conduct or grades, the repayment program
begins 3 months after they have dropped out from their proposed
program. The application form will reflect this contract between
the student and the Scholarship Fund Committee.
The Scholarship Fund Committee will meet every 6 months to review
the status of students receiving funds. A bill will be sent to students
no longer in medical-related programs. Depending on the amount of
the students award, the repayment agreement will reflect payments
distributed over a period of one year. The Scholarship Fund Committee,
and the hospitals controller, will administer the repayment
process.
Publicity Process
A news release will be sent to all newspapers in the San Luis Valley
and to all local radio stations. A letter of information and application
forms will be sent to all high schools in the San Luis Valley during
the months of March/April each year.
