Who
can attend?
Diabetes affects not only the person with diabetes, it also affects
their support systems: family members, care givers, significant
others and friends. Our participants are encouraged to bring
support persons with them at no additional cost. To allow for adequate
seating please call 587-1309 to make arrangements.
Up to one year from the initial appointment, the self-management
education program participants are invited to attend FREE refresher
classes or individual appointments.
However, if blood sugars begin to get out of control consistently
<70 mg/dl or >180 mg/dl or the HbA1c increases please
call 587-1309 to set up the refresher appointment.
What
is the self-management education schedule?
The initial individual education assessment is scheduled as soon
as the referral from the primary health care provider is received.
Ideally, the initial appointment is within 10 days of the referral.
If group education is appropriate, specific group classes
are held at different times to meet individual schedules. Morning,
afternoon and evening group classes are available. We schedule appointments
so you can complete your education within 4 months of the initial
referral. The first group class is usually the food and nutrition
class. Additional appointments are based on individual learning
goals or medical needs.
Who are the Instructors?
The Diabetes Education Empowerment Program provides a multi-disciplinary
approach to help the person with diabetes and the family members learn
how to manage the disease. Our team is directed by your primary
health care provider.
With
appropriate diabetes self-management , your quality of life can
be very enjoyable. Complications from diabetes can be controlled
and may often be prevented. Diabetes self-management education with
personal empowerment means diabetes does not control you
you
learn to control your diabetes!
