DEEP - Diabetes Education Empowerment Program

Our goal is working to empower individuals for a healthy, successful life with diabetes. We are dedicated to assist you in preventing diabetes or preventing the complications of diabetes and slowing down possible damage from your disease process.

The diabetes education program at SLVRMC is recognized by the American Diabetes Association. We provide standardized individualized diabetes education in both group and individual settings, based on the individual needs of the person with diabetes and the family.

The content components include nutrition, prevention of both chronic and acute complications, blood glucose monitoring, insulin pump program, individual goals which include quality and length of life, and free follow-up for a year from the first date of service, after completion of the education program.

Our team has a passion, and a commitment to provide the best education to our clients affected by diabetes. Staff with personal and family experience with diabetes makes our team quite unique. We have expertise in weight management and exercise, medication options, current advancements in technology, and strategies to control blood glucose levels.

The central location at the SLV Regional Medical Center in Alamosa increases accessibility to the entire San Luis Valley. There are sattelite clinics in LaJara and Del Norte. However, all appointments are made through the San Luis Valley Regional Medical Center office at 587-1309.
