DEEP - The Program

  • Individual Assessment of Learning or Educational Needs and Personal Goals.
  • SBGM — Self-Blood-Glucose Monitoring - instruction for accurate testing, including assistance for acquiring supplies at low or no cost — utilizing insurance.
  • Instruction for insulin self-administration.

Diabetes Self-Management Education includes:

  • Nutrition and exercise.
  • Individualized meal plan.
  • Disease process and types of diabetes; Type 1, 2 and GDM
  • Medications (oral and insulin)
  • Interaction of exercise, blood glucose levels, food, stress, and coping strategies.
  • Sick day management.
  • Recognition and treatment of low and high blood glucose levels.
  • The American Diabetes Association’s Standards of Care
  • Setting personal goals and identifying community resources.

What are the program benefits?
With appropriate diabetes self-management , your quality of life can be very enjoyable. Complications from diabetes can be controlled and may often be prevented. Diabetes self-management education with personal empowerment means diabetes does not control you… you learn to control your diabetes!

Why do I need Diabetes Education?
Diabetes is a chronic, metabolic, medical condition that can affect people of all ages. When diabetes is well controlled, there is reduced risk of complications. If diabetes is poorly controlled there are increased risks of complications, including:

  • Heart disease
  • Stroke
  • Blindness
  • Kidney failure
